Dynamic Forms

ClinicYou has ability to create electronic forms and send them for patients to fill in. Currently it is accessible from the patient medical records - this might change in the future.

he 'Send Form' icon will appear when the patient has an email address. Click the icon and you will see the popup which allows you to create forms or to send a form to the current patient. If you are going to the form building page, you will find a very intuitive form building UI to create forms.
