Can I Create Custom Forms and Send Them to Patients?#



Yes! Here is how:



Go to a patient's medical records page (EMR), and click the 'Send Form' icon, which will be visible when there is an email address for the patient (this will not be visible without one).

In the resulting popup, it is likely that you will see 'No Forms Found' if it's the first time you are trying this. Click 'Create Forms'. In the resulting page, follow the instructions on screen to create a form, or multiple forms. The new form you are creating will be automatically selected in the 'all forms' panel and you can begin to add questions. You can reorder questions and any changes are automatically saved.



All forms you create will be available in the popup on EMR and you can then send them to the currently selected patient.

Once the patient fills in the form and submits it, the questions and answers will be automatically filed as a Note within the medical records.

A form sent to a patient can only be filled in once. Subsequent attempts to submit the same form will fail. You can of course resend another version of the same form if you wish (say to track progress of pain management for instance).
